Details of the consent for the use of my image

I give my consent free of charge and without any time or quantity limitations. The consent covers fixing, processing, alteration, reproduction of the materials with my recorded image and their distribution through any medium (any form of image and sound transmission), in accordance with the purpose and activity of the Foundation, in particular to promote the programmes (projects) run by the Foundation without their prior approval.

The Foundation is also entitled to transfer the materials containing my recorded image as well as my personal data to the extent necessary for identification (name and surname, institution) to entities with which the Foundation cooperates in order to promote, use and disseminate the results of the Foundation's activities. I also declare that the use of my image in accordance with this consent does not violate my personal interests or rights.

By giving the consent, I acknowledge that:

1. the Foundation is the Data Controller (and data also includes the image processed on the basis of the above consent). 
2. Personal data will be processed only for the purpose specified above in the consent.
3. My image will be processed for the period specified above in the consent.
4. Data is protected by technical and organisational measures to guarantee an adequate level of protection in accordance with applicable legislation.
5. The Data Controller will process my image in order to fulfil the purpose described in the consent above pursuant to Article 6(1)(a) and will not collect data from third parties.
6. Data may be shared with:
   1. companies providing to Data Controller IT, accounting, administrative and other relevant services.
   2. Institutions managing programmes implemented by the Foundation, in the scope that it is indispensable to carry out these programmes.
7. I have the right to:
   • obtain information about the processed data, including the categories of processed data and possible recipients of data;
   • require the rectification of incorrect data or have incomplete personal data completed;
   • obtain from the Data Controller the erasure of personal data or restriction of data processing, in accordance with the provisions of GDPR;
   • object to data processing, in accordance with the provisions of GDPR;
   • receive personal data provided to the Data Controller in a structured, commonly used and machine-readable format and transmit such data to another controller;
   • lodge a complaint to the supervisory body – The Inspector General for Personal Data Protection (Generalny Inspektor Ochrony Danych Osobowych, ul. Stawki 2, 00 - 193 Warsaw - if You find that your data is illegally processed.
8. I have the right to object to the processing of any and all personal data at any time. Such an objection does not affect the legality of data processing conducted prior to the objection.
